Tech neck is a description of a posture that somebody turned into a diagnosis. There is no such condition in any classification of disease. There is a common posture — head forward, chin down, eyes on a screen — and there is common neck ache, and the phrase asserts a link between them that the research has repeatedly failed to find.
That does not mean your neck does not hurt. It means the explanation you have been given for why is weaker than the ache is.
The short answer

What tech neck is
The claim runs like this: looking down at a phone or a low screen tilts the head forward; a forward head multiplies the load on the neck; multiplied load over years causes pain and eventually damage.
Each step sounds obvious. The first is a description and it is true — people do put their heads forward at screens. The second is a physics calculation. The third is an epidemiological claim, and it is the one nobody has established.
The sixty pounds came from a model
Hansraj published the number in Surgical Technology International in 2014: a static biomechanical model of the cervical spine, loaded at increasing angles of head tilt, giving about 60 pounds of force at 60 degrees.
That is a real calculation and it is not a finding about health. It measured no person's neck and followed no person's pain. It has been repeated so widely — in news pieces, in posture-product marketing, in physiotherapy clinic waiting rooms — that it now reads as an established clinical fact, which it never was.
The arithmetic is also less alarming than it sounds when you say what it describes. Your neck holds your head up all day at whatever angle you are using. Load on a structure is not damage to it; muscles and joints are load-bearing by design, and the same logic applied elsewhere would make sitting down a hazard because it loads the hips.
Grade: not evidence about pain at all. The figure is fine as biomechanics and worthless as prognosis.
What happened when somebody looked
Damasceno and colleagues, in the European Spine Journal in 2018, took the obvious step: they measured text neck posture in young adults aged 18 to 21 and looked for the association with neck pain. They did not find one.
Richards and colleagues, in Physical Therapy in 2016, had done the larger version for sitting posture — more than 1,000 adolescents, sorted into distinct posture clusters by measurement rather than by eye. The clusters were real. The clusters did not differ in neck pain the way the model predicts.
Grade: reasonably solid for the negative in young people. Two independent samples, measured rather than judged, both looking for the association the phrase asserts, neither finding it.
The counterweight, at full strength
The honest version of this page has to include the finding that goes the other way, because it exists and it is the best thing the posture side has.
Mahmoud and colleagues, in Current Reviews in Musculoskeletal Medicine in 2019, meta-analysed the relationship between forward head posture and neck pain. In adults, they found an association — small to moderate. In adolescents, they found none, which fits Richards and Damasceno.
Read it carefully, because the shape of the study limits what it can say. These are cross-sectional measurements: posture and pain recorded at the same moment. A person whose neck hurts may hold their head differently because it hurts. An association measured at one instant cannot tell you which came first, and no trial has shown that moving an adult's habitual head position reduces their pain.
Grade: moderate for the association in adults, absent for the causal claim. Forward head posture works through the measurement side — including how badly people judge their own.
So why does my neck ache at a screen?
The better-supported explanation is duller and it does not need a posture theory.
A screen day is a sustained low-level muscular effort in one position for hours, with almost no movement variety. Static loading of that kind produces discomfort in laboratory conditions in people with no complaints at all, and the discomfort resolves when the position changes. That is why a break feels good immediately and why the relief does not last: you changed the exposure, not the tissue.
It also explains why the intervention with the best evidence in this whole area is not a stretch. Andersen and colleagues, in Pain in 2011, gave office workers with frequent neck and shoulder pain two minutes a day of progressive resistance training and measured a real reduction over ten weeks. Their earlier work in Arthritis & Rheumatism in 2008 found specific strength training for the painful muscles outperformed general fitness training, and Blangsted and colleagues' one-year trial in the Scandinavian Journal of Work, Environment & Health in 2008 found the same pattern.
If your neck hurts at work, that is where the evidence points first. Stretching is for the stiffness of a held position, and the two problems get run together constantly.
What to actually do
- Change position often. Every 30 to 60 minutes, for a minute or two. The break research is weak but consistent and the cost is nothing. The micro-breaks research sets out what it does and does not show.
- Raise the phone rather than lowering the head, if it is comfortable. Not because 60 pounds is coming for you, but because holding one position for an hour is the exposure, and a different position is a break from it.
- Move the neck through its range — chin tucks, side tilts, a rotation each way. Eight desk-viable movements with their holds are listed in desk stretches.
- Add a very small amount of resistance training for the neck and shoulder muscles. Two minutes daily has a randomised trial behind it; no stretch does.
- Stop reading about posture. The evidence does not support the effort it invites, and posture correction exercises grades that claim in detail.
When it is not a posture question
Most neck ache at a screen is unremarkable and settles. Some of it is not, and the list below is short and worth knowing.
See a clinician who can examine you if there is pain radiating down an arm, numbness or pins and needles in the hand, weakness in the arm or grip, neck pain that followed a fall or a whiplash injury, pain that wakes you at night, unexplained weight loss, fever, or any disturbance of balance or coordination. What we can and cannot measure

Limber has no neck test. The battery is five tests and the neck is not one of them; the neck area score is inferred from the shoulder reach test at full weight, and the app names it as a proxy rather than presenting it as a reading. The thoracic spine — the segment underneath the neck that a screen day stiffens most — is fed by a seated rotation test that is not built yet either.
What you can measure today is the shoulder mobility test, the Apley scratch, both sides, five bands, which reads the chain the neck sits on top of; and the Range Score, which composes whatever you have taken and draws the unmeasured ground as unmeasured. That is four of five stations at best, and every page that composes one says so.

Is tech neck a real condition?
Not as a diagnosis. It is a name for a posture, and the implied causal link to neck pain has not held up: a study of text neck in 18 to 21-year-olds found no association with neck pain, and a larger study of sitting neck posture clusters in over 1,000 adolescents found the same. Neck pain is real; tech neck is an explanation for it that keeps failing its test.
Does looking down really put 60 pounds on your neck?
That figure comes from a static computer model of the cervical spine published in 2014, not from measuring anybody. It is a plausible calculation of force at an angle and it says nothing about whether that force causes harm. Your neck is load-bearing by design, and load is not damage.
Can tech neck cause permanent damage?
There is no good evidence that it does. Nobody has shown that habitual screen posture produces structural change, and imaging findings in the neck are common in people with no symptoms at all. Persistent or severe symptoms are worth examining on their own merits rather than as a consequence of your phone.
What are the symptoms of tech neck?
The phrase is usually applied to aching or tightness across the neck and upper shoulders, sometimes with headache, after a long screen session. Those symptoms are common and non-specific. The ones that matter more are radiating arm pain, numbness, weakness or a change in coordination — see a clinician who can examine you if any of those are present.
What actually helps neck ache from screen work?
Two things with different evidence behind them. Changing position often relieves the discomfort of a held position immediately, on weak but consistent evidence. And a very small daily dose of resistance training for the neck and shoulder muscles reduced pain in randomised office-worker trials — that is the better-supported intervention and it is not stretching.
Does raising my monitor fix it?
It changes the exposure, which is worth something, and the trial evidence for specific workstation adjustments is thin. A Cochrane review of ergonomic design and training found little evidence that such interventions prevent upper-limb and neck disorders. Raise the screen if it is more comfortable; do not expect the height itself to be the answer.
